To assist other practitioners within the primary care mental health service to triage referrals, assess, plan, implement and evaluate individual care plans.

To cover the role of duty for both the Open Door Team and the Neighbourhood mental health team and offer advice to the general public, GP’s and other professionals

The post-holder will contribute to the provision of the duty role.
The post will involve triaging referrals and assessing the appropriateness for the primary care service, and signposting to other services if not appropriate.
This post will be responsible for the assessment and formulation of presenting mental health problems experienced by people at a primary care level.
You will also be responsible for delivering brief psychological based interventions for people experiencing common mental health problems
This post involves liaison with other roles and services within the community.

Tameside and Glossop Neighbourhood Mental Health Team offers advice signposting and support to individuals who require support with their mental health. The Open Door is the central point of entry into mental health services for Tameside and Glossop residents over the age of 16. The team receives all routine (non - urgent) referrals into mental health services. The team offers a triage function and, if required, completes routine assessments to identify the most appropriate care pathway.
