**We are seeking a local care leader to join our team on a part-time basis, based in Tonbridge.**

Our Operational Team Leader will work closely with all users of our services and their families, friends, and carers. Our Team Leader will be expected to be kind and responsive, but professional and informative and contribute to the quality of the service provided by keeping service users as safe as possible through sound clinical skills and effective risk assessment, ensuring the best possible outcomes for people at home.

We would like our Team Leader to support the growth of our service, conduct supervisions and 1:1s with our team, check in regularly with our clients and their families, but also deliver face-to-face care and support, cover gaps, and provide some flexibility. Our Team Leader will work as an active member and leader of our team, guided by our standard operating procedures and protocols. Our Team Leader will develop specialised skills and knowledge in the work we do, and in providing supervision and in the training and appraisal of team members.
- **Service user safety**_
- Developing practice to meet core skills, and working within competency framework relevant to the area
- Maintaining at all times a kind, caring and professional attitude towards service users, relatives and colleagues, taking into consideration dignity, privacy, and confidentiality, cultural and religious needs
- Safeguarding people by recognising and responding when an adult or child might be at risk from abuse but also recognising their own limits and asking for help and escalating concerns when necessary
- Escalating safety concerns and by doing so acting as effective advocates for those who use our services
- Being open and transparent about own practice
- Working with others to create a culture of continuous improvement
- Maintaining accurate records
- Maintaining compliance with statutory and mandatory training in addition to all applicable competencies
- Demonstrating adherence to the Code of Conduct for Healthcare Support Workers and Adult Social Care Workers in England.
- **Clinical outcomes**_
- Supporting service users to maintain their independence and confidence by taking an enabling approach to providing person-centred care and support
- Encourage and promote service user involvement in planning and delivery of care, maximising independence
- Work with, and in support of, all other colleagues and stakeholders involved in the care of service users, including allied health professionals, nursing colleagues, and third-party providers, commissioners, and support networks
- Visiting new service users to introduce self and the service
- Assisting service users with all aspects of personal care and hygiene, promoting dignity, respect, and privacy at all times
- Support service users with meal preparation and monitoring of flood and fluid intake
- Assistance with medication management, maintaining appropriate record sand ensuring the highest standards of medication safety
- Undertake moving and handling operations in-line with manual handling legislation
- Working with other team members to ensure that plans of care are carried out to the agreed standard
- Sharing good practice in order to improve standards of care
- Acting as service user advocates in the multidisciplinary team
- Identifying emergency situations and taking appropriate actions
- Being responsible for own delegated tasks
- Conducting care and support assessments, reviews, and handling new enquiries and referrals
- Monitoring care outcomes and service quality.
- **Service user experience**_
- Working in partnership with service users and their families and carers
- Gaining consent and, as far as possible, involving people in all decision-making
- Signposting service users and carers to registered practitioners to further support their care
- Reassuring people by being professional, responsive, knowledgeable, and confident
- Investigating and resolving concerns and issues raised by junior staff, or escalating to seniors as necessary.
- **Supporting yourself and others**_
- Participating constructively in own supervision and annual appraisal processes
- Being responsible for own personal development, including keeping up to date with statutory and mandatory training and training relevant to any specialised practice
- Identifying own development needs and taking action to enhance own knowledge and skills and areas of competence, considering organisational needs and objectives
- Reviewing and reflecting on own competencies and performance through effective use of operational supervision and appraisal and identifying any personal training needs or skills gaps
- Sharing best practice in relation to own area of expertise with colleagues and external parties where beneficial, acting as champion for own specialism and supporting colleagues in delivering care appropriate to national guidance, service user preference a
