This role will include flexible working and fast paced changes. As a support worker, you will play a vital role in providing practical and emotional assistance to individuals in their own homes whilst assessing their needs

**What will a day in the life of a Support worker involve?**
- Working closely with people, their families and other professionals to create care plans tailored to their needs and goals.
- Assisting and enabling individuals with activities of daily living, including bathing, dressing, meal preparation medication management, and mobility support.
- Collaborating with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
- Monitoring and evaluating individuals progress, adjusting care plans as needed to promote optimal recovery and well-being.
- Maintaining accurate and thorough documentation of interactions, care plans, and progress.

**To be a successful Support Worker, what will you need?**
- Previous experience in a similar care, preferably within a healthcare or social care setting.
- Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals lives.
- Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
- Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
- Ability to work independently, demonstrating initiative and reliability.
- Flexibility to work various shifts, including evenings, weekends, and bank holidays.
- A full driving licence and access to a vehicle is preferred but not mandatory.
