**We’ve got an opportunity**

An exciting opportunity has arisen for an experienced outreach worker to join the British Red Cross in Northern Ireland, working on an innovative NHSE programme that supports people who access health care more than expected (High Intensity Use (HIU) Programme).

Based in the community across Belfast and South Eastern Trust areas, you proactively engage and intensively work with a rolling caseload, helping to identify and address unmet social needs that may be exacerbating physical and/or mental health. The people we support may have complex presentations, including mental health, substance misuse, multiple long-term conditions, adverse traumatic experiences, and they are often suspicious of formality and offers of support. So, we are looking for someone whose ability to connect is exceptionally strong. You must be authentic, relatable and compassionate.

A large part of our role involves lowering the stigma that is associated with this cohort and to change mindsets and behaviours that can adversely affect access and recovery. By building strong relationships with partners, such as housing teams, the local voluntary sector and statutory partners, you will advocate for access and improve integrated working.

By focussing on strength-based interventions, you and your team will improve individuals’ self-management techniques, leading to improved engagement with other services and improved wellbeing and confidence. There will also be a heavy focus on care co-ordination and addressing health inequalities, so you will need to know how the system works and how to help people who are falling through gaps. Person centred support planning and dynamic risk assessing are a core part of the role too.

Lastly, most of your time will be spent out in the community; meeting people in their homes, in cafes or walking in the park, so you will also need to be confident of lone working procedures and able to keep meticulous support records whilst on the hop. You will also need to be physically able to maintain face to face activity (within public heath guidelines).
- Due to the specific requirements of this role, there is a need to visit various locations, some of which are remote with no access to public transport, therefore you must have access to a vehicle which you are willing and able to use in conjunction with your duties or be eligible for transportation via the Access to Work Scheme._
