We are looking for motivated staff to join our developing IOH service. The service incorporates:

- Liaison & Diversion - assesses vulnerable individuals with complex needs being brought into the criminal justice system; including but not limited to support with mental health or a learning disability, alcohol or substance misuse, homelessness, financial needs or social difficulties.
- Mental Health Treatment Requirements - Supports the mental health and wellbeing through access to primary level psychological treatments
- RECONNECT - seeks to improve the continuity of care of vulnerable people leaving prison by working with them before they leave and supporting their transition to community-based services.

Those who come into contact with the criminal justice system are far more likely to have unmet health and social care needs. By understanding and addressing those needs, we can reduce reoffending and help our service users on their recovery journey. The IOH Navigator role is a new innovative post, embedded within the custodial, prison and community pathways of the service. Offering screening assessments for a range of vulnerabilities and supporting those individuals to engage with the right services to meet their needs.
- Independently undertake face to face and electronic screening to support the identification of multiple vulnerabilities of people who have been in contact with the criminal justice system or immigration centre, such as police custody, courts, prison and community settings. Working to guidelines.
- Work within limitations and escalate to IOHP as appropriate.
- To support IOHP, STR Workers and Lived Experience workers and Admin as necessary.
- Provide an embedded service in police custody suites, courts and prisons working within legislation requirements, such as PACE (1984).
- Work as part of a team with the freedom to work independently, particularly in relation to providing advice to service users, carers, and professionals who contact the service.
- Work with service users in conjunction with other professionals, to devise and implement effective individualised programmes of support.
- Liaise with other agencies as required dependant on the person’s presentation, work closely with other agencies, such Custody and Prison Health Care Professionals, and community agencies.
- Actively promote the service to external agencies as a positive example of user led services.

MPFT provides physical and mental health care, learning disability, and adult social care services across Staffordshire, Stoke-on-Trent, Shropshire, Herefordshire, Worcestershire and Telford & Wrekin. In addition, we provide services on a wider regional and national basis. We serve a population of 1.5 million and employ around 8,500 members of staff.

What we ask from you

Working closely with the multi-agency IOH team, you will receive referrals for individuals in the IOH population and you will;
- Triage referrals
- Assess service users’ individual needs.
- Complete and review risk assessments and risk management plans
- Offer liaison, joint-working and referrals to other relevant services
