**Job overview**:
We are looking for an experienced, caring, motivated Assistant Practitioner to join our busy, professional and dynamic team. You will need to be able to demonstrate a real enthusiasm for community nursing as well as a desire for ongoing professional development.
You should also have an understanding of the changing climate of care delivery in the community.
The ability to travel independently is essential for this role.
**Main duties of the job**:
We currently have a vacancy for a full time Assistant Practitioner working 37.5 hours per week.
We are looking for a motivated, positive and enthusiastic individual who will be required to join our proactive and innovative nursing team. Applications are invited with evidence of training, previous experience and the ability to work autonomously, whilst being a member of a team committed to excellence in patient care. If you have excellent communication and people skills and would enjoy the challenge of caring for people at home, then we want to hear from you.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Over 4,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.

Just over 532,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon.
