**JOB TITLE**:Health and Wellness Social Leader

**LOCATION**: Locality based within North Tyneside

**ACCOUNTABLE TO**: Charity Services Manager

**REPORTING TO**: Health and Wellness Lead Coordinator

**PURPOSE**:
The post holder is responsible for the delivery of social activities as part of the wider community based Health and Wellness Programme for an ageing population across a variety of settings in North Tyneside. The post holder will deliver a programme of Health and Wellness activity sessions at selected venues throughout the borough.

The post holder will provide a number of weekly classes to improve the health and
Wellness of older adults. This post holder will be managed by the Health and Wellness
Lead Coordinator for the service and will work alongside other Social Leaders, the
Trainers and the Coordinating Officer.

**DUTIES AND RESPONSIBILITIES**:
1. To deliver a programme of social activities sessions at selected venues throughout the borough.

2. To screen and assess the capacity of those people participating in group sessions, tailor and adapt sessions as appropriate and monitor progress

3. To actively encourage participation in the community and communicate effectively with participants to promote long term adherence and progression of social programmes, eliciting and recording participants’ feedback

4. To ensure volunteers that are involved in the Health and Wellness Service are inducted, supervised, supported and developed under the guidance of the Charity
Services Manager

5. To maintain effective attendance registers and up to date participant records in accordance with relevant legal and organisational requirements

6. To record and collate output, outcome and quality measures in accordance with project requirements

7. To support the Health and Wellness Lead Trainer and Charity Services Manager by providing information for monthly reports

8. To provide care and dignity to participants at all times, whilst taking into account individual needs
9. To act as an Emergency First Aider should the need arise (training available if required)
10. To maintain up to date training and membership qualifications for appropriate bodies, in line with the delivery of the role and the programme and personal and professional development

11. To deliver Health and Wellness promotion talks to groups in the community and provide appropriate information which aims to increase participation in the service’s activity programme

12. To assist with the delivery of Health and Wellness events across the borough
13. To develop and sustain effective relationships with all stakeholders to promote the safety, independence, health, well-being and personal development of people in later life in North Tyneside

14. Other duties as directed by the Charity Services Manager which are consistent and commensurate with the responsibilities of the post and the needs and development of the Health and Wellness Service.

**Customer Responsibilities**

1. To hold ‘guided conversations’ and social/Wellness support planning with individuals to identify a programme of community support to achieve personal outcomes and an improved quality of life

2. To review customer needs on a regular basis to ensure goals are still relevant and achievable

3. To Provide support/signposting where required to achieve goals
4. To build strong relationships with the Customer Services Team to ensure awareness of all services and the organisation and its partners has to offer

**Compliance Responsibilities**

1. To work in line with stakeholders and Age UK North Tyneside’s Policies and

Procedures as relevant to the role, including GDPR
2. To support management of the project budgets in line with the organisation’s financial procedures
3. To ensure all outcomes and outputs for the projects are achieved and reported in line with funder and organisational requirements
4. To monitor Health and Safety compliance of volunteers and customers, reporting any problems or deviation from health and safety best practice to the HowFit Lead Trainer and the Charity Services Manager

5. To ensure that Age UK North Tyneside’s Equal Opportunities policies, principles and practices are observed and implemented throughout service delivery

6. To work with the Health and Wellness Lead Coordinator and the Charity Services
Manager to ensure quality systems are upheld, monitored and reviewed
