Perinatal Mental Health Service Practitioners

To work with the other staff in the service to triage, assess and care plan for women presenting with perinatal mental health problems To include women, their families and carers in decision making and care planning To signpost or refer patients to appropriate statutory and non-statutory services To work closely with the external stakeholders to promote a good care experience for perinatal women To conduct routine and emergency biopsychosocial, risk and mental health assessments in accordance with service policy To safely discharge patients from the perinatal mental health service in collaboration with other perinatal mental health service staff To negotiate clinical input from other disciplines from within and external to the perinatal mental health service to ensure the provision of a clinically appropriate and multidisciplinary approach To develop close working links with other mental health teams and agencies to enable swift and appropriate transfer between services To contribute to the development of robust MDT care plans in consultation with patients, families/carers, community teams, primary care, childrens services and other appropriate service providers To work in line with Safeguarding Children and Adult policies and procedures, including participation in safeguarding meetings when required To participate in clinical supervision both on an individual basis and team/peer supervision (e.g. Reflective Practice) To be a source of specialised perinatal mental health knowledge for any person requesting this, e.g. midwife, health visitor, support worker, carer To assist in the writing and delivery of training to other NHS colleagues, voluntary organisations and other groups as required To offer support and guidance to other team members To contribute to the teaching and support of students as required Maintain clinical records according to professional and Trust standards To attend and contribute to clinical and team meetings within the perinatal mental health service To contribute to delivering interventions to support recovery To work flexibly and provide cover in the absence of other clinicians within the perinatal mental health service To provide comprehensive written reports as required
