Do you want to work in a friendly, professional and patient-focused Community Mental Health Team where your skills, knowledge and experience would be valued?
We are after a band 6 mental health practitioner to join our team as a “ Key Worker” to work with client’s over 65 years of age with a diagnosis of an Organic Illness. There would be times that you may also be involved with clients with a functional diagnosis to aim towards their recovery goals. You would be working as an autonomous practitioner completing assessments. The successful applicant would be responsible for assessing care needs and the development, implementation and evaluation of proposed interventions.
This is a full time position based at Jocelyn Solly Resource Centre. The service operates 9am-5pm, Monday to Friday and considerations can be made for flexibility. You would be joining a well-established, experienced and supportive multi-disciplinary team that is always looking to evolve and adapt to changing practices in mental health care. The position includes actively engaging in service planning and development as well as focusing on personal growth and development.
To work within the Adults and older Adults Community Mental Health Team within a defined geographical area. The post holder will be responsible for triaging, completing new assessments. Undertaking assessments and coordinating the care of service users. Ensuring that communication with service users, families and GP is of a high standard. To develop and maintain positive working relationships with all involved Ensuring that clear channels of communication are established and maintained.
To promote and identify areas requiring development within the CMHT in the Cheshire East area and to take a lead role in the development of services in conjunction with the Team.
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.
New entrants to the NHS will commence on the minimum of the scale stated above.
