We have a fabulous job opportunity for you to join our Hospital Discharge and Crisis Team as a Lead Support Worker

You’ll be uniquely placed within the hospital multi-disciplinary teams, tasked with helping patients, from the point of hospital admission to discharge, to overcome housing related barriers.

You’ll continue this support when customers move on into their own home, and will be integrated into the crisis team, providing housing related support to help reduce readmissions. This is a fast paced, exciting service with lots going on, but it’s a real opportunity for you to get stuck in and change lives.

**About you**

You’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.

To make sure that we keep our records straight, and customers safe, it's important that you are technologically savvy (or at the very least not a technophobe). You’ll be given the latest IT kit supported by our new online integrated IT system. This means that you don’t have to be in the office as you can complete support plans on the go. We will always be on hand to help, if you need it

As this is a community-based job, you’ll need to travel around to spend time with your customers and your team. You could do this using public transport, or if you drive and have access to a vehicle that would be amazing (we pay your expenses to cover this use).

As you’ll be working within NHS wards, all colleagues must be fully vaccinated against COVID 19.

- When you join as a Lead Support Worker, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one._

You’ll be working alongside our fabulous team of 5 Lead Support Workers and our Operations Manager Sarah, who are all passionate about supporting customers grow in their independence and reach their goals. No two days are the same, and our support is always centred around the customer as an individual.

**Working hours**

It’s our unwritten rule that you attend the important appointments in life, be it your child’s school play, your partner’s graduation, or to get your new fridge delivered. You can elect to fit these around your working day or use your flexi to take the time off. Our focus is on getting the job done, not your working hours.
