**Patient Care Coordinator Primary Responsibilities**: To act as a key point of communication between the Practice and GP Practices within the Primary Care Network and other healthcare organisations. Working to the standards and founding principles set by the Practice. Provide caring, un-biased and timely support to patients and other individuals accessing the Practice. Input data into the patients healthcare record as necessary.
Answer incoming phone calls, transferring calls or dealing with the callers request appropriately. Maintaining and monitoring the practice appointment system. Keeping patients informed of their appointments should they be delayed or cancelled. Process personal, telephone and e-requests for appointments.

Signposting - receive, assist and direct patients in accessing the appropriate service or healthcare professional. Initiating contact with and responding to, requests from patients, team members and external agencies. Read code data on EMIS Web.
Photocopy documentation as required. Data entry of new and temporary registrations and relevant patient information as required. Input data into the patients healthcare records as necessary. Direct requests for information i.e.
SAR, insurance/solicitors letters and DVLA forms to the administrative team Manage all queries as necessary in an efficient manner. Processing incoming mail and electronic mail as appropriate. Carry out system searches as requested. Maintain a clean, tidy, effective working area at all times.
Monitor and maintain the reception area and notice boards. Support all clinical staff with general tasks as requested. Maintaining the efficient patient records system, including filing and following requests. The list of duties in the job description should not be regarded as exclusive or exhaustive.
