**Job Purpose**:
We are currently looking for a compassionate and professional individual to complete our team, who provide early intervention services to service users and families living in the community, focusing on the direct needs of young people experiencing a first episode of psychosis.
The post holder will work with their own caseload of 14-35 year old experiencing a First Episode of Psychosis from within the ABUHB EIP service, providing recovery focussed early intervention support with a focus on clients social, emotional, and vocational recovery, in collaboration with and complimenting the clinical interventions provided by ABUHB EIP practitioners.
**Main Duties and Responsibilities**:
With the support of the team, the post holder will assist in the provision of care and treatment to clients aged 14-35 in a community setting, in line with the principles and practices of engagement, social inclusion and recovery.
Support
- Providing practical social, emotional and vocational support for young people aged 14-35 following a diagnosis of a psychotic illness.
- To enable clients to exercise informed choices about their lives where possible and to enhance their social skills and participate in the community
- Provide a recovery focused service to individuals experiencing a first episode of psychosis, to ensure clients are aware of their rights and responsibilities within the project and society.
- Support individuals through a tailored support programme with a view to move-on to community engagement, vocational opportunities using Adferiad Recovery’s Recovery Programme as the basis for doing this
- Supporting individuals through a range of activities, on an off-site and peripatetic basis.
- Promoting, encouraging, and facilitating the use of local community services.
- Identifying and meeting the needs of individuals to enable them to maintain their health and wellbeing through engaging individuals in programmes such as Adventure Therapy / Outdoor activities / Physical Health promotion groups and social peer groups.
- Providing information, advice, and guidance as appropriate and in a timely manner.
- Assisting individuals in identifying areas of support and identifying the best option of support in line with their Care & treatment plan through the use of Dialog+ QOL outcome measure
- Providing services on a one-to-one casework basis.
- Providing services on a group basis.
- Completion of individual support/recovery plans, exit plans and other documentation as required.
- Support and educate carer/wider family members, signposting and referring for additional support where appropriate.
External Collaboration
- Communicate clearly, effectively and professionally with individuals receiving service, Support Workers, Carers, family members, referrers and other Professionals via multiple formats (i.e. face-to-face, telephone, in writing etc.)
- Signposting/referring on to other specialist/appropriate services in a timely manner.
- Producing reports for internal/external purposes as required, supporting the Health Board with audits and service reviews.
- Maintain records and statistics required to monitor and evaluate the effectiveness of the service in line with Adferiad and the ABUHB systems.
**Essential Qualifications**:
- There are no specific qualifications but there must be evidence of commitment to personal and career development relevant to the post.
- An NVQ in Care Level 3 would provide some evidence of competence for the post and is therefore desirable, along with Coaching / Teaching and A Level qualifications.
**Essential Experience**:
- Must have experience of providing care and support services to people, including young people, families and carers, with a serious mental illness
- Must have experience of working as part of a multi-disciplinary team, preferably within a clinical setting
- Experience of working with individuals affected by a diagnosis of a First Episode Psychosis
- Experience of working within an Early Intervention model of recovery
- Experience of delivering family intervention
- Experience of working with service users within mental health
- Experience of delivering/supporting indoor/outdoor activities on a 1:1 or group basis.
**Other Relevant Requirements**:
- Full driving licence and full access to a vehicle for work purposes
