We are looking for a CRHT Support Worker to be part of an established Crisis resolution and home treatment team, supporting patients who are acutely mentally unwell in the community, and providing the best level of care to them whilst remaining in their own homes. We provide support for individuals who are experiencing an acute mental health episode in the community, as well supporting people who have been discharged from inpatient wards. We provide a menu of interventions, to ensure the highest level of care and support is delivered.
This is an opportunity to be part of a dynamic service, providing opportunities to develop your skills and your career. You will play a key role in the multi-disciplinary team. You will need experience of working in a mental health care setting and have skills and experience in delivering a menu of interventions aimed at promoting recovery such as: health promotion, anxiety management, social inclusion, emotional first aid, self harm reduction and safety planning in collaboration with patients, carers and social support networks.
This is a permanent role at agenda for change band. The service covers a 24 hour shift rotation over 7 day’s a week. The team cover the Canterbury, Thanet and North East Kent localities.

1. Contribute in the assessment of clients referred to M&S CRHT.
2. To provide support to clients of M&S CRHT and their carers in relation to personal care, daily living and social activities, and help maintain the social network by the use of a range of home treatment interventions.
3. To carry out specified interventions, alone on occasion, as per crisis plan and update as appropriate.
4. To monitor progress of CRHT clients against the crisis plans and communicate findings, recommend changes to senior M&S CRHT staff in order for crisis plan to be reviewed.
5. Work flexibly in order to respond to crisis work and adjust allocated work and prioritise at short notice as required by Shift Co-Ordinator.
6. Engage clients and carers with M&S CRHT crisis plans and negotiate options available whilst promoting crisis prevention recovery and resilience.
7. Support carers to enable clients to remain at home. On occasions assist client with administration of medication as per crisis plan.
8. Promote work of M&S CRHT to clients, carers, other mental health professionals, statutory and non statutory agencies
9. To communicate with members of local mental health services, other mental health professionals to allow consistent and comprehensive care to be provided to clients.
10. To contribute to the collation of data and audit of performance and practice.
