**Job overview**:
Dementia Intensive Support Team - West Suffolk
An exciting opportunity has arisen to join the West Suffolk Dementia Intensive Support Team. You will be joining a motivated and passionate multi-disciplinary team working to achieve positive outcomes for service users and their carers.
The team is community based and works in individuals homes to prevent breakdown of care and support, to promote independence and stabilise situations preventing admissions to hospital and premature moves into long term care. You will be holding a caseload and taking on the care coordinator role.
If you have the same passion to work with this client group, can demonstrate strong communication skills and have experience in liaising and coordinating with multiple professionals then we would love to hear from you.
**Main duties of the job**:
Working with other multi-disciplinary professionals within the team you will:
- Manage and hold a caseload and act as care coordinator for service users you are seeing
- Undertake initial assessments, manage risks, be positive in risk taking
- Engage and work with clients with dementia and their carers
- Liaise with statutory and voluntary Sector to get care and support in place as required
- Support introduction of care and monitor situations

The Dementia Intensive Support Team is a part of the West Suffolk Care Group, working within the community to meet the needs of people with Dementia and their carers.

**Detailed job description and main responsibilities**:
They will be undertaking initial assessments, demonstrating strong communication skills and an ability to engage with clients and their carers often in difficult circumstances.
They will be contributing to multi-disciplinary team meetings and liaising and working with other agencies in the statutory and voluntary sectors to establish packages of care to meet the service users and carers needs moving forwards.
They will be assessing risk on an ongoing basis but be open to positive risk taking with this client group.
