This new post provides an exciting opportunity for a qualified and experienced mental health nurse, social worker or occupational therapist to work alongside community mental health teams in Wakefield to enable and ensure smooth transitions from Children's to Adult Mental Health Services for Young People and their families.
The role of Young Persons Transition Lead MH Practitioner will involve clinical leadership, line management and supervision responsibilities as well as assessing mental health needs and delivering direct interventions with Young People.
