**Job description**

Experience with working with vulnerable adults/in a health and social care setting is desirable. Ideally a Level 2 qualification in health and social care or care certificate. All mandatory training will also be provided and must be completed and updated according to health and social care standards. The Support Worker will support service users in accordance with service user’s individual person-centred support plans, with the overall aim of supporting our service users to stabilise their lifestyles, develop self-care and living skills, support them with activities of daily living and where appropriate accommodation based support.

**Key Duties.**
- Provide general emotional support and counselling.
- Promote effective communication for and about individuals.
- Relate to and interact with individuals.
- Promote, monitor & maintain health, safety and security at work.
- Promote choice, well - being and the protection of all individuals.
- Liaise and facilitate with external agencies for additional input, support and feedback (probation, drug workers, doctors, social workers etc).
- Advice and assistance with fulfilling contractual obligations, including the payment of utilities, help maintaining safety and security of the home.
- Assist service users with benefit claims and ensure full uptake & claims are kept running.
- Accompany service users to appointments, including shopping support.
- Support service users to manage finances effectively and develop budgeting skills.
- Participate in a key worker system and support service user to develop skills and meet desired outcomes.
- Practical help and skills development in all aspects of daily living, including self care and living skills, shopping, cooking and dietary advice, cleaning.
- Participate in general service user observation and report any changes to senior staff.
- Detection and prevention of conflict and crisis with appropriate measures utilised.
- Support to source and set up independent accommodation through a planned resettlement programme.
