The post holder will be responsible for working in Liaison and Diversion (L&D) Services, with the flexibility of working across Staffordshire, dependent on skill and service need. L&D is not itself a treatment service, but it is an identification, assessment and short term support service.

There will be Liaison and Diversion Practitioners within Custody Suites and Court setting to identify and assess vulnerable individuals. It uses assessments to make appropriate referrals for treatment and support, and ensures criminal justice practitioners are notified of specific health requirements and vulnerabilities of an individual which can be taken into account when decisions about charging and sentencing are made.

The post holder will be responsible for providing short term support to vulnerable individuals upon leaving Custody or Court, where further support is required. The post holder will be responsible for working closely with the individual to prioritise need and plan actions to address identified need. This may include supporting an individual to make a referral to a specific service and support the individual to attend the initial appointment with that service.

The role will involve ongoing assessment, accessing and implementing a wide and, at times, complex range of care services across an ageless Staffordshire Service.

The post holder will be responsible for carrying out assigned tasks as part of the Liaison and Diversion Service.

1. Working with service users referred to Liaison and Diversion and the individual’s carers, helping to promote self-determination, choice and autonomy.
2. To effectively establish a therapeutic relationship with service users and relatives / carer.
3. Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
4. Assess patient's individual care needs, agree and refer on to appropriate agency. Involve the individual in the planning and prioritisation of their activity where possible. Provide short term intervention to encourage individual to engage with services and attending their initial appointments.
5. Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
6. Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition
7. Utilise agreed outcome measures to review the effectiveness of process, as part of Liaison and Diversion Service.
8. Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
9. Assist in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled and to identify factors which may inhibit these from being achieved.
10. To attend relevant clinical meetings / forums as requested.
11. To plan and manage own delegated workload on a day to day basis.
12. To support with the planning induction of students and new staff.
