Band 6 Community Rehabilitation Occupational Therapist: Hunter AHP have an opportunity for an experienced Community Rehab Occupational Therapist to secure an ongoing locum contract, in a, community environment. The service provides rehabilitation for patientswho have recently been discharged from local hospitals. These patients will have experienced conditions such as falls, rehab post de-conditioning from acute, rehab post infection such as UTI, LRTI, COPD management, PD management. There caseload will be sensitiveto individual needs within the resources available.

Community Rehab Occupational Therapists work as part of a wider multi-disciplinary therapy team, as this role is community based a car driver will be required.

**Key responsibilities of the Community Rehab Occupational Therapy role as follows**:

- To provide a high-quality Occupational Therapy service, managing and prioritising a caseload of service users who are on early support discharge from hospital.
- To work as an independent clinical professional with well-developed knowledge and skills, problem solving, clinical reasoning and clinical expertise in specialist area.
