This is an exciting opportunity for someone who wishes to develop specialist Paediatric Diabetes Dietetic Band 6 skills within a small and supportive team (cross site). We are looking for a self
- motivated, friendly and enthusiastic Dietitian to join our established Children’s Therapy Team across Northern and North East Lincolnshire working for Northern Lincolnshire and Goole NHS Trust.

We are a small but dynamic multi professional team comprised of Dietitians, Physiotherapy, Occupational Therapy and Speech and Language Therapy. The team is based in the community across both Scunthorpe and Grimsby sites.

Your job will be to provide specialised Paediatric Diabetic Dietetic assessment and management of children aged 0-19 years with within community and inpatient arena for children with complex presentations. There will also be an opportunity to develop Paediatric Diabetes Dietetic skills and competencies, which will be fully supported through training if you have not completed this previously. You will be part of the wider Diabetes MDT, contributing to service development, evaluation and development of pathways and will have a key role in delivery of Education and training to both parents/carer’s/young people and to other professionals. You will work alongside our current diabetes trained dietitian in order to meet the needs of the caseload. You will also have access to supervision from the Diabetes Team our partner Trust, Hull University Teaching Hospital (HUTH) and the ability to access the Yorkshire and Humber Diabetes Network for ongoing support.

As part of this role you will be expected to provide line management to other registered and non-registered staff. You will work closely with the established team; Team leads and colleagues across Health and Education. You will also support the team lead(s) in new service development activities regular audits and development of Outcome measures.

Clinical Duties
1. Be professionally and legally accountable for all aspects of your own work. Ensure a high standard of clinical care for patients under your management, supporting more junior staff to do likewise
2. Accept clinical responsibility for a designated caseload of patients and organise this effectively and efficiently across site and within inpatient and outpatient/community settings
3. Undertake comprehensive assessment, treatment and discharge of patients using advanced clinical reasoning, knowledge of evidence based practice, investigative and analytical skills incorporating training and practical experience
4. Engage service users in treatment planning decisions, in line with their individual goals. Implement individual or group interventions in collaboration with the patient using graded activity to achieve therapeutic goal
5. Evaluate patient progress, reassess and alter treatment programmes at each contact as required, using standardised outcome measures where appropriate
6. Provide advice, teaching and instruction to patients, relatives, carers and other professionals, to promote understanding of the aims of treatment, and to ensure a consistent approach to patient care
7. Organise discharge/ transfer arrangements for continuing support if required and ensure transfer of patients to out of area/ other dietetic teams when required. Where this includes the need for prescribed products and/or enteral feeding pumps with associated equipment, ensure that correct resources are ordered and supplied
8. Participate in Diabetes MDT meetings/clinics to ensure the delivery of a co-ordinated multidisciplinary service and to integrate therapy treatment into the overall treatment programme. This will include discussion of patient care, patient progress and discharge planning
9. Provide Diabetes advice, teaching and training to other members of the MDT, parents/carer’s and other professional

13. Be responsible for maintaining accurate, comprehensive patient treatment records in line with Trust and professional standards
