- Role is mainly **Office-based** in Westbury-On-Trym, Bristol

Premier Homecare's team of **3 Homecare Coordinators **(**Note: - NO rota planning responsibilities)** play a critical role ensuring our clients feel well supported every single day. We'd like to expand this team.

We have found what makes a great Homecare Coordinator is someone who is highly organised, can deal with pressure, can communicate well with all sorts of people - both verbally and written someone who can work confidently and well as part of a team. Customer Service is key within the team. Those who succeed build up wonderful relationships with their clients, their families along with colleagues and find the role incredibly rewarding.

**Duties/Responsibilities**:

- Working with Homecare Coordinator colleagues, support the Registered Manger in the successful delivery of the best possible care to clients
- To undertake client home assessments
- Produce, set up and thereafter review as necessary, all client care related documentation such as care plans & risk assessments
- To ensure accurate entry of data on all required systems
- Liaison with client family members regarding their loved ones care provision
- To work closely with and liaise with health & social care professionals - GP's District Nurses, Occupational Therapists, Hospital staff to ensure joined up care for clients
- To Liaise with Premier Homecare's professional care workers and other field care staff to ensure understanding of clients care needs
- To ensure compliance with all regulatory requirements, contributing to positive CQC inspection results
- _(The role _**DOES NOT**_ _involve rostering or scheduling duties)_
- This position is normal office hours **Monday to Friday**

**Key Attributes / Skills & Experience**
- Proven track record of working in the care sector
- A good understanding of Social Care and relevant CQC regulations
- Exceptional interpersonal & communication skills; able to develop and maintain positive relationships with Clients’, their friends and families, along with health care professionals such as social workers, occupational therapists, district nurses, hospital staff and doctors
- Ability to prioritise effectively
- Planning and organisational skills
- Enhanced administration and IT skills
- Ideally qualified to Level NVQ 3 or above
