Health Care Assistant for Specialist Nursing

Job Introduction
We have an exciting **part time** role as a **Health Care Assistant **within our Bickerstaff House, Ormskirk team. This is a flexible role for **22.5 hours** to work closely with our Community Nursing Teams.
Our work is crucial to the community as we aim to prevent unnecessary hospital admission. You will support and assist the senior team members to provide care to people in their own homes for a wide range of conditions including diabetes, respiratory, cardiac and continence care.
Main Responsibilities
- To work as a part of a team to provide a service to deliver Support to patients in their own homes.
- Use enhanced skills to facilitate effective communication for patients with aphasia or other barriers to communication and understanding.
- Monitor patient’s progress, modifying intervention and reporting observations back to Senior clinicians within the teams, highlighting variations to expected outcomes from treatment programmes.
- Assist nurses in assessment procedures and provide equipment within agreed protocols.
This is an opportunity to put your NVQ level 2/3 or equivalent/appropriate qualification/experience to great use. However, this is not essential.
In this role, you will need a **driving license** and access to your own vehicle, allowing you to travel to service users' homes in the West Lancashire area.
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
