An exciting opportunity has arisen for a Band 7 Community Mental Health Team (CMHT), Team Lead within Lower Monmouthshire Adult Mental Health Services. The role will involve line management and clinical leadership for the Community mental health team based at Hywel Dda Chepstow. We are looking for an individual who is enthusiastic and motivated in providing evidence based patient care to a complex patient group with a range of mental health difficulties within the lower Monmouthshire catchment area.

The post holder will ensure the comprehensive provision of a cost effective, accessible, appropriate and responsive mental health service to service users with complex mental health needs. They will support the Senior Nurse, Local Authority and Health Board personnel with the implementation of the Mental Health National Service Frameworks, underpinned by Recovery and develop policies and protocols to support the associated principals and values.

The post holder will carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance. The Community Team Manager will develop and coordinate a multi-professional approach providing evidence based mental health care to all those who use the service and ensuring that this collaboration harnesses a integrative agency approach so that physical, psychological and social care agencies are working together in providing holistic care to individuals who require its support.

The post holder will ensure service delivery is of a high professional standard that is in line with Health Board policies and procedures.

Your role would enable well organised, safe and effective care that is recovery focused. The care should be person centred with clearly identified clinical outcomes/ targets. You would need to have significant experience of working with mental health services and have developed management skills that would be transferable to this role.
