**What is The Deal for you ?**
- **Flexibility**You can work your full time hours over 4 days and enjoy a long weekend, or work over 5 days to accommodate your other commitments.
- **Hybrid Working** Do you like to work from home? Or do you prefer being in an office? With this role you have the option of working from home or from our regional offices - or a blend between the two
- **Salary** £36,000 - £38,000 + £3,000 car allowance

**Who you’ll be supporting & more about the role**:
As a Complex Care Assessment Practitioner, you will lead the assessment and development of personalized care plans for individuals with learning disabilities and mental health issues. Your role will involve:

- **Person-Centered Assessments**: Conducting thorough assessments to identify individual needs and outcomes.
- **Collaboration**: Working closely with clinical teams and external professionals to ensure comprehensive care.
- **Quality Assurance**: Leading quality evaluations and ensuring compliance with regulatory standards.
- **Advocacy**: Being a strong advocate for the individuals we support, ensuring their voices are heard.

**Essential Criteria**

To be successful in this role, you will need:

- **Education**:Minimum of level 5 qualification in PBS, and willing to work towards a level 6/7 as part of the role
- **Experience**:Proven experience in care and support assessments, particularly with individuals with learning disabilities and/or mental health issues.
- **Knowledge**:Understanding of Positive Behavior Support, Transforming Care Partnerships, and relevant regulatory environments.
- **Skills**:High-level communication skills (written and oral), strong IT skills, and the ability to manage complex assessments.
- **Competencies**:Ability to maintain focus during assessments, manage emotionally stressful situations, and build strong relationships with stakeholders.
- **Qualifications**:Trusted Assessor qualification (England only) and experience working with Local Authorities and public sector organizations.
