Discharge Coordinator

This is an exciting opportunity to play an integral role in supporting a seamless patient pathway from admission to discharge.
You would be working with a friendly and dynamic team who are dedicated to providing excellent service to our patients and stakeholders by facilitating safe and timely discharges.
The post holder will have responsibility for actively participating as a key member of the multi-disciplinary team and have a leading role on discharge planning.
The post holder will have an extensive knowledge of community services and will support development of new initiatives that support timely discharge.
The post holder will support the wider team in identifying barriers to discharge and problem solving to overcome them to achieve positive results for the patient, their family, and carers.
Supervise the ward based patient flow coordinators.
The post holder will work across several wards. The wards are spread over two hospital sites, Bodmin Hospital and Longreach House.
