TFS Healthcare is seeking experienced ward based, highly skilled, and qualified Band 7, 8a and 8b PCN Pharmacist's for new, rewarding opportunities.

**Duties will include**:

- Medication Reviews
- Clinical Medication reviews for long term conditions
- Medicines optimisation
- Repeat prescribing authorisation
- Medicines reconciliation
- Clinical Audits
- Meds safety alerts
- Shared responsibility for Budgets, and high risk drugs management.
- Networking with local and National pharmacy network for patient care and improve outcomes and share ideas of good practice and improve patient safety and care

**PCNPharmacistjob description**:

- Wards based role
- UK experience is a must
- Some flexibility to work from home
- Flexible hours are considered

**As a PCNPharmacistyou will need to hold the following requirements**:

- To be registered with the GPhC
- Experience in an NHS setting
- UK experience is a must
- Must have GP Practice experience
