Community Phlebotomist

- Integrated community team are looking for skilled individuals with phlebotomy experience to join us to work within our community nursing service. We are looking for a Band 2 Phlebotomist. You will be part of a team delivering a high-quality service to housebound patients supporting referrals from our local partners.

We are a cohesive team who are supportive, caring and passionate about delivering excellent patient care. As a phlebotomist you will work closely with nursing colleagues and the referral centre to deliver a phlebotomy service Monday to Fridays.

Main duties of the job

The role of the phlebotomist is to provide a service to support community nursing within the Integrated Community Team (ICT). To ensure patients are able to access timely venepuncture as a result of an appropriately completed request from a clinician.
- Demonstrate an awareness of physical, psycho-social, cultural and environmental needs of patients and their family/carer when in patient's homes
- To obtain high quality blood samples from patients in the home environment
- Responsible for planning day to day work tasks and activities within an occasionally unpredictable environment
- To work independently and as part of a team.

The role of the phlebotomist is to provide a support service to the community nursing service within the Integrated Community Team (ICT). To ensure patients are able to access timely venepuncture as a result of an appropriately completed request from a clinician.

This role will provide domiciliary phlebotomy in appropriate patient’s homes for patients who are on a community nursing caseload. The role involves working as a lone practitioner in patients’ homes.

To perform designated venepuncture procedure to patients in the community.

Ensure patient and personal safety through adherence to Trust procedures, protocols and relevant health and safety legislation at all times.

Participate in training, supervision and mentoring of new and less experienced phlebotomists.

Ensure collection of blood request forms from base and GP surgeries prior to home visits.

Transport samples to the appropriate GP surgeries safely and in a timely manner.

Work largely unsupervised to duties as assigned by the District Nurse / Senior Community Nurse who will review performance and undertake appraisal.

Maintain accurate records on the clinical system (SystmOne) to comply with trust requirements. Work will be allocated onto an electronic ledger on SystmOne.

Inform the community nursing team regarding any unsuccessful attempt at venepuncture.

Ensure phlebotomy kit is well stocked and orders are requested via the community nursing admin team.

Any concerns regarding patient presentation or home circumstances must be escalated to the Referral Centre manager and documented on SystmOne.

Ensure patient safety through adherence to the Trusts Non-access policy.

As a Health Care Assistant/ Support Worker you will be responsible for participating and assisting in the delivery of care / services to patients and service users as part of a Mental Health and Social Care Health Service. Health Care Assistant, Social Care Workers and other Healthcare Support Workers employed by the Trust are required to follow the guidance set out in Code of Conduct for Health Care Assistants/ Health Care Support Workers to provide safe and compassionate care of a high standard, and to challenge others who are not.

You will receive direct supervision and partake in reflective practice / training on a regular basis and be required to meet the Care Certificate standards. All successful applicants will also have the opportunity to discuss the prospect of undertaking a Level 3 Healthcare Support Worker Apprenticeship alongside their substantive post. This apprenticeship would include the Level 3 Diploma in Clinical Healthcare.
