An opportunity has arisen for an Independent Prescribing Pharmacist with Meds - Management experience to join a PCN in Coventry, working at different surgeries within the locality. They can offer part-time or full-time hours. ASAP Start, 6-month contract.

**About the Role**:

- Help support the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation
- The post holder will take responsibility for areas of chronic disease management within the practices and undertake clinical medication reviews to proactively manage patients with complex polypharmacy
- 15 - 20-minute consultations, via telephone/face-face
- EMIS Practices
- 6 Month locum opportunity
- ASAP start
- Part-time or Full-time hours available
- £40 - £48 p/h dependant on experience
- Based in Coventry
- Onsite Parking

**About the Individual**:

- Independent Prescribing Pharmacist (V300 Qualification)
- Experienced in the Primary care Setting
- Possesses good knowledge of QOF, QIPPP, Auditing, and NICE
- EMIS Experience
