Floating Support Officer

As a Floating Support Officer, you’ll be working with the Floating Support team to deliver high quality support. You’ll help customers who are vulnerable and/or have complex needs, living in their accommodation in either the North or South of the Cheshire East borough.

**Job requirements**
- Work with customers to create a support plan to work through their individual needs.
- Lead person-centred and confidential support meetings with customers on your caseload to help them achieve outcomes on their support plan.
- Empower customers to maintain their independence and understand what it means to manage their own home.
- Support customers to develop positive relationships with their support network and other services.
- Work closely with the Floating Support Team Leader to promote Concrete’s services and build positive relationships with the local community.

**What we’re looking for**
- Educated to GCSE level or with a good level of maths and English.
- Experience working in a customer facing role.
- Great communication and problem-solving skills.
- Strong organisation and time management skills, with the ability to prioritise cases.
- Full UK driving licence with own vehicle for work purposes.
- DBS check.
