Overview:
We’re looking for a passionate and committed individual with a particular interest in working with high intensity users of hospital, mental health, 999, GP and Primary Care Networks, where support for alcohol and/or drug use has been identified as a need. If you’re looking to:
- Create simplified services for people with multiple and complex needs
- Work within multidisciplinary teams, ensuring that clients receive the right care at the right time
- Deliver services within a range of health and social care settings
- Be part of a team at the cutting edge of new ways of working
Then this might be the ideal role for you
**Responsibilities**:
The successful applicant will have a background in health and social care who has:
- A professional qualification in health and social care or proven relevant experience
- Ability to undertake comprehensive assessments and formulate robust recovery and risk plans
- Experience in the delivery of psychosocial interventions to both groups and individuals
- Good IT skills, able to create and maintain accurate records and high quality written reports
- A flair for developing productive multidisciplinary relationships
