Assistant Practitioner - Band 4 - Mildenhall Community Healthcare Team - 37.5 hpw - Full Time - Permanent**

An Assistant Practitioner post, Band 4, full time, 37.5 hours per week, is now available within the Mildenhall Community Healthcare Team.

We are looking for an applicant with appropriate experience in a variety of NHS clinical areas though previous community experience is not essential.

Our team of District & Community Nurses, OT’s & Physio’s work with the GP practices in Mildenhall, Lakenheath & Brandon, along with our Community Matron, to provide high quality patient centred care, alongside Alliance partners - ACS, Home Care, Council & Voluntary sector colleagues.

We have 2 office bases - one in the new Mildenhall Hub, alongside ACS colleagues & also at the newly combined Brandon Leisure & Health Centre.

We receive referrals for adults with long term conditions; patients discharged from acute hospitals with ongoing needs and in the prevention and management of falls with referrals from Ambulance services. Treatment is carried out in patients own homes.

The post holder is expected to have excellent communication & IT skills & be able to provide evidence of working successfully at Band 3 level with PT & OT competencies already completed.

The post holder will need to be able to travel to rural locations to meet the needs of the post. Pool vans are available for staff to use on a shared basis & to transport equipment.

**For Band 1 and Band 2 posts, the cost of the DBS Disclosure will be met by the Trust. However, any employees at Band 1 and Band 2 who leave the Trust within six months of commencing their post will have a £10 administration fee deducted from their final salary. For posts at Band 3 and above, the cost of the Disclosure will be met by the appointed employee and deducted in instalments from their salary over a period of three months. If the employee leaves the Trust within three months of commencing their post, any outstanding payments will be deducted from their final pay.
