**About the role**

This role involves working across Shropshire Mental Health Community Services providing care and support to individuals with diagnosed mental health needs and empowering customers to sustain long term achievements**.**

**What you’ll be doing**
- Deliver and promote a high standard of support for each individual in their home to include demonstrating and advising on good practice.
- Promote individual customers’ independence and choice.
- Advising and assisting in developing a range of social, leisure, work, education or other meaningful activities as appropriate with customers.
- Support customers to take an active role within the community
- To support customers to maintain or establish relationships with friends and family
- Undertake initial and on-going assessments in consultation with the customer, manager and involved agencies.
- To ensure that all of the services recording logs are maintained clearly and accurately.
- To maintain individual customer’s record as determined by the line manager.
- To participate in making written records as per legal and regulatory requirements and as determined by the manager.
- To complete accident/incident/near miss reports in line with policy and procedures.
- To undertake training as required by the service.
- Adhere to Trident Policies & Procedures.
- To work on a rota basis covering days, evenings, weekends, and bank holidays as required.
