Barnet, United Kingdom Jewish Care Full time

We are seeking a proactive Community Mental Health Practitioner to join our Intake team and provide front-line support for clients reaching out to Jami. This key role involves triaging referrals, signposting to external services, undertaking needs-led assessments, and delivering short-term interventions. This role will also support our step-down service, which offers lighter touch contact for clients transitioning out of active 1-1 support.

**About the Role**:
As a Community Mental Health Practitioner within the Intake service, you will be a key contact for individuals seeking advice and support, as well as the first point of contact for those interested in Jami services. You will conduct needs-led assessments for entry into Jami services, occasionally delivering short-term interventions where our 1-1 support is less appropriate. Working within our step-down service, you will provide light-touch monthly phone calls to those transitioning out of 1-1 support.

**Key Responsibilities**:
Intake Services:

- Triage and manage incoming referrals to our duty inbox in a timely manner.
- Conduct needs-led assessments via in-person, telephone, or video platforms.
- Identify and address safeguarding concerns in line with Jami’s policies.
- Develop relationships with Jami’s service teams, holding relationships with key individuals in these teams
- Appropriately know of, and signpost to, external services where appropriate.
- Provide well-informed advice on mental health legislation and available resources.

Step-Down Services:

- Manage a caseload of clients with potentially complex needs, who are transitioning out of 1-1 support.
- Deliver light-touch support, including monthly phone calls, to these individuals.
- Monitor client progress and adapt care plans to meet their evolving needs, including escalating concerns back into 1-1 support when appropriate.

General Duties:

- Collaborate with Jami’s multi-disciplinary team to deliver integrated mental health services.
- Maintain accurate, up-to-date client records.
- Work efficiently and sometimes independently on Microsoft outlook platforms programmes.
- Ensure compliance with safeguarding, health and safety, GDPR, and other organisational policies.
- Participate in supervision, appraisal, and continuous professional development.

**About You**:
We’re looking for someone with a passion for mental health, excellent communication skills, and the ability to thrive in a dynamic, fast-paced environment.

**Essential Criteria**:

- Recognised qualification in health or social care (e.g., Social Work, OT, RMN) or 5+ years’ experience in a relevant role.
- Proven experience in needs-led assessments, risk management, and safeguarding.
- Strong interpersonal, organisational, and IT skills.
- Knowledge of recovery principles and mental health legislation.

**Desirable Criteria**:

- Lived experience of mental health challenges.
- Familiarity with resources available within the Jewish community.

**Why Join Us?**:
At Jami, we offer a supportive and inclusive work environment where you can grow both professionally and personally. You’ll have the opportunity to make a tangible difference in people’s lives while being part of a passionate, dedicated team.

Jami is here for when mental illness and distress make everyday life a struggle. In November 2023 Jami and Jewish Care announced they would become one, to ensure long-term sustainable services for people of all ages in the community. With the combined expertise of Jewish Care and Jami, we can support even more people in their journey to better mental health, providing treatment, support and education to young people, adults, families and community leaders in the Jewish Community.
