Rheumatology Patient Monitoring Assistant Administrator - Band 3
The post holder will be the point of contact for all administrative issues relating to Rheumatology patients’ pathway of care for remote blood monitoring. The post holder will work as part of a multi-disciplinary team to ensure the remote monitoring of patients on a remote monitoring pathway is managed smoothly. The post holder will also work closely with the Rheumatology CNSs and Consultants to support robust monitoring of the patient remote monitoring of their condition and act accordingly to the specialty instructions and SOPs. To work closely with the DAWN/Biologic Multi specialty Remote Monitoring Administrator for Integrated Medicine and closely with the Rheumatology Clinical Administration Team (CAT 9) to ensure appropriate cross-cover is provided.
Process and log all DAWN/Biologic referrals for the Rheumatology specialty, in line with Trust Access and Administration Process Policy and the approved Trust Rheumatology SOP.
Ensuring that capacity is proactively and efficiently used. This includes booking patients into the right clinic to ensure that they are seen by the most appropriate clinician first time and for subsequent follow ups.
Ensure that any diagnostics and investigations that are required as part of the patient pathway have been requested and that the results are available for consultations.
Ensure outcomes of clinic attendance have been accurately recorded on the DAWN/ Biologic system, for all patients on the remote monitoring pathways.
Complete Biologic and Dawn referral and update the administration team.
