The **Admission Prevention Team **is being expanded across Norfolk and we are looking for enthusiastic and creative people who enjoy a challenge to come and join us.

What does it involve?

You will be going out to meet people in the community who are finding it difficult to manage due to a deterioration in their mental health and who may well be on the cusp of a hospital admission or have recently been discharged from hospital. You will recognise that even in the most difficult of situations, people are often relying on a range of strengths and skills to cope and survive. You will work alongside them, proactively helping them to identify the things that will help them to stay well in the community. You will use your ability to connect with people and develop their trust to help them to quickly navigate the system and overcome barriers.

**Tell me a bit more about the job?**

The Admission Prevention Community Team operates across Norfolk 7 days a week, including bank holidays, between the hours of 09:30-20:00. You would be working on a 7-week rolling rota, flexibility is welcomed and working hours/shift patterns can be discussed at interview. The service has two localities: the first being South and West Norfolk and the second locality being Norwich, North Norfolk and Great Yarmouth and Waveney. We are currently recruiting to both localities within the service and locality preference will be discussed at interview. Although it is expected the support you provide will be within your allocated locality there may be occasions you are expected to work across Norfolk depending on demand. A typical day within this role would consist of visiting vulnerable adults with mental health needs in the community to provide a variety of support including, emotional support, social inclusion activities, signposting and supporting to engage in services in the community. You will be working within a friendly and knowledgeable team and will receive a comprehensive induction when you join us as well as training through our annual cycle.
