**Salary**:Up to £24,000 (FTE)

**Start date**: March 2021

**Summary**: You will be supporting families whose children have cancer or a life-challenging illness providing practical and emotional needs-led support services, with the main aim being to maximise the opportunities and support available to them.

Working with St Richard’s, Chichester and Worthing Hospitals you will establish and grow our Momentum caseload and facilitate our Support Services in each of the hospitals whilst building a collaborative partnership with the medical teams.

We’re looking for someone with a background in Family Support, who understands the importance of timely, meaningful, empathetic and non-judgmental support for all ages.

**Background**

Momentum Children’s Charity supports families whose children have cancer or a life-challenging condition. The charity was founded 17 years ago by our current Chief Executive and has grown enormously over that time. We have partnerships with nine hospitals, in SW London, Surrey and Sussex, who refer families to us for our support, ranging from therapies to respite and special family experiences.

Our vision is to reach many more families in need of our support and we are looking for people to join our team with passion, enthusiasm and dedication to help us continue to grow.

You will be an excellent communicator who enjoys working with people and who is looking for their next challenge Working closely with the Senior Family Support Worker during the initial phase, we will ensure that you have the support needed to deliver our Momentum service as reflected in our other hospitals.

**KEY DUTIES AND RESPONSIBILITIES**
- Establish Momentum support service in allocated hospitals (St Richard’s, Chichester and Worthing)
- Provide timely, meaningful, compassionate and non-judgmental support to all ages within allocated caseload of families, and report back on needs and recommendations for support referring to a series of relevant therapies and services, where needed
- Work within the Family Support Team, to handle allocated caseload of families, and report back on needs and recommendations for support
- Implement the use of ‘Outcomes Star’, a tool used to measure our impact as a service and our work with families
- Highlight grandparents in need of support and signpost to our grandparent service
- Transition bereaved families across to our Echoes Service
- Weekly visits to hospitals engaging with both Momentum and non-Momentum families in need of support
- Referral of child/siblings/parents to a series of relevant therapies, where needed, to Family Support Coordinator
- Input, update and maintain accurate records on database in line with Momentum’s requirements
- Management of family referrals within agreed catchment area with assessment and delivery of ongoing support
- Keep regular contact with key personnel within the paediatric team at the hospitals to encourage and facilitate prompt updates on both new and existing patients
- Delivery of family support services in line with the budget and the overall charity Business Plan
- Plan and manage family support events relevant to your caseload alongside Family Support Coordinator
- Support the marketing and fundraising team through the provision of family updates, photographs and case studies
- Relay supporter contact information to appropriate internal contacts
- Attend Family Support Team meetings to share family information, discuss issues and recommendations, review processes for support, and monitor outcomes/impact
- Attend relevant training appropriate to the post of Family Support Worker
- Flexibility to attend family support events, which may involve evenings or weekends
- Undertake regular supervision
- Report any safeguarding concerns to Momentum Safeguarding lead in a timely manner
- Act as a professional representative of the charity for both internal and external audiences
- Work within Momentum’s Safeguarding and Code of Conduct policy
- Have a good level of internet access at home to enable remote working where required
- Other ad hoc duties in line with your role
