Support Worker Band 3

We are looking for a dynamic, motivated and enthusiastic person who has excellent communication, IT literate and problem solving skills alongside a passion for patient centered care. You will work corporately within our Primary Care Team North and will be supporting the Community Nurses, Occupational Therapists and Physiotherapists providing high quality care to housebound residents who live with a wide range of physical and mental health conditions.
You will work generically across the nursing and therapy service as needed in order to provide a holistic approach to those you visit in their residential environment, this will include nursing aspects such as clinical assessments/ monitoring and reporting and equipment provision for rehabilitation programmes. You will be part of a MDT team in which you will receive clinical and managerial support.
You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams.
- To assist trained staff and carry out duties delegated by them in line with their training in the provision of quality evidence based practice to clients.
- To assist in maintaining the smooth running of the team and contribute to the development of the services.
- To work within the multidisciplinary team in a primary care setting including patients’ homes, residential homes and clinics, under mínimal supervision.
- To provide concise feedback to registered nurses regarding patients’ well-being and any changing needs.
- To maintain accurate record keeping, and carry out administrative tasks.
