Band 4 Diabetic Failsafe Officer

**PLEASE NOTE**: This post is based at Prince Charles Eye Unit, King Edward VII Hospital, St Leonard's Road, Windsor, SL4 3DP - and attracts a High Cost Area Supplement (HCAS) of 5% of gross basic salary.

The Ophthalmology Department is seeking a passionate, enthusiastic, reliable and hardworking individual to join our team as the Diabetic Failsafe Officer.

You will be the main point of contact for administrative issues relating to Diabetic patients’ pathway of care, assurance that all patients’ pathways are managed using national assurance standards and local clinical decisions.

You will work as part of a multi-disciplinary team and will provide expertise in the proactive management of the Diabetic patients’ pathways from referral to discharge, ensuring the entire pathway is managed smoothly.

You are also expected to work closely with the National Diabetic Eye Screening Programme, and build a good working relationship with their administrators and Failsafe Officers.

This is an exciting time to join the Ophthalmology Department as we move towards more digital ways of working, including virtual clinics and assessment.

We are also able to offer flexibility in working from either of the 2 main Ophthalmology hubs at Reading and Windsor, as well as working from home on rotation.

**The post-holder will**:
1/ Be point of contact for all administrative issues relating to diabetic eye patients’ pathway of care providing assurance that all patient pathways are managed within national assurance standards and local clinical decisions.

2/ Work alongside a multi-disciplinary team and will provide expertise in the proactive management of the diabetic patient pathways from referral to discharge, ensuring the entire pathway is managed smoothly.

3/ Be the key point of contact between the Hospital Eye Service and local Diabetic Eye Screening service.

4/ Ensure high levels of patient and clinician satisfaction by being an accessible, customer-focused and knowledgeable point of contact. The role will provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.
