Patient Keyworker

Are you interested in being part of an exciting initiative focusing on discharge pathways from the point of admission to ensure a seamless transfer out of the acute environment, giving the patient and/or family the best possible experience of this often complex pathway.

So if you are a band 2/3 with the drive and enthusiasm to extend your own skills whilst also working with our teams to improve our current discharge services, we would love to hear from you.

This role will give the successful applicants the chance to be ward based whilst also working closely with the Discharge Services and ward Multi disciplinary teams on the inpatient wards aiming to facilitate prompt and timely discharge planning as part of the discharge to assess model.

Successful applicants will be supported by the hospital at home service and as such there will also be an opportunity for career progression with the completion of competencies relevant to this role at band 4, car driver will be essential for band 4, as this role will follow the patients out into the community.

The positions will be across all wards and operate over 7 days, so there will be weekend working on rotation.

The post holder will also act as a conduit between the ward based team and the discharge teams and in doing so will provide an extension of the discharge team based within the ward environment. The post holder will also be expected to attend weekly Multi-Disciplinary Team (MDT) with the discharge team and weekly MDT on the allocated ward and length of stay meetings.
