An exciting opportunity has arisen to join the Swansea Community Integrated Therapies service in the Swansea Bay University Health Board. The successful postholders will work in a variety of community
- based settings across Swansea Bay working alongside Physiotherapists, Occupational Therapists, Speech & Language Therapists in addition to other support worker colleagues.

The post holders will provide rehabilitation and discharge planning to patients and groups of patients in a number of settings, including patients’ own homes and care homes, step up & down reablement bedded facilities and community exercise classes. The post holders will support therapy input across the community and Home First pathways. The post holder will be able to adopt a flexible approach to work and will also be expected to participate in 7 day working rotas. You will need to be able to demonstrate experience of working in rehabilitation in NHS, Social Care or Community based settings. Previous therapy assistant experience is highly desirable for this role. An existing NVQ 3 /AGORED qualification/ evidence that you are working towards the physiotherapy/occupational therapy support worker diploma or other relevant Healthcare or exercise diploma is essential,
The role will also entail some administrative responsibilities, so good basic IT knowledge and/or understanding of healthcare IT systems is highly desirable.
You will also be expected to provide support to the physiotherapy assistants and reablement staff.
You will be supported by qualified therapists and the clinical team leads.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

To be a fully engaged member of the integrated therapy team Team providing a service to the specific clinical area.
To undertake assessment and intervention programmes with individual clients as directed by a senior Therapist underpinned by appropriate experience/knowledge. The post holder will also carry out delegated duties with groups or individual clients, as identified by the Therapist. This includes supervising and instructing clients / clients in activities relating to their treatment programme. The role may involve the fitting and instruction on use of adaptive equipment, the assessment and supervision of adaptive daily living skills and leading therapeutic groups. The post holder may be required to carry out any of these
tasks single
- handed. The postholder will be responsible for assessing and providing interventions to allocated clients in a number of different settings as directed by the Therapist. These include the clients own home,
Gorseinon Hospital, Bonymean House and Ty Waurnallwydd and other residential and nursing homes across Swansea.
To be responsible for assessing allocated client following discharge from hospital at the request of a Therapist, in order to ensure that they are coping in their home environment, to provide and demonstrate equipment, progress mobility as part of the Community and Home First pathways.
To work alongside colleagues reablement and community settings in order to facilitate optimal packages of care and an individual’s independence.
Carry out administrative and clerical duties, in relation to departmental/service needs and maintenance of client records (paper and electronic) and statistical information
