**The Transfer of Care Hub (TOCH)** is a co-located, hospital-based team consisting of the relevant services (acute, community, primary, social care, and specialist services including housing, voluntary sector, etc) to coordinate the discharge of Croydon residents from Croydon University Hospital (CUH) into the appropriate discharge setting.

The cornerstone of this team is the ‘Integrated discharge team’, a ward-based team with a wide range of skillsets and backgrounds including nursing, therapy, and social work. The team will co-ordinate all aspects of supported discharges and as the ‘single point of discharge’ from CUH. The team includes roles where traditional professional boundaries and responsibilities have been blurred, to streamline discharge processes and improve experiences for patients and staff. The TOCH is currently in its early stages of development, building on the existing Integrated Discharge Team (IDT).

Croydon Health Services NHS Trust and Croydon Council are looking for enthusiastic and dynamic individuals to join the Integrated Discharge Team within the Transfer of Care Hub. You will be primarily responsible for working closely with ward teams on designated wards to agree comprehensive discharge plans for patients, perform assessments for discharge and to coordinate an effective, smooth, and timely discharge into community settings.

The Discharge Facilitator role (along with Senior Discharge Facilitators) will be an integrated ‘trusted assessor’ role meaning the post-holder can have different professional backgrounds including nursing, social work, therapy or without professional registration. This will enable Discharge Facilitators to perform a wider range of tasks which would previously have been performed by health staff or social workers separately.

As the role supports partnership working, good communication, inter-personal skills and technical ability are fundamental to the role. You will work with a range of colleagues within Croydon University Hospital and Croydon Council, of varying seniority and experience. You will use your experience and expertise to support the effective running of the Integrated Discharge Team thus enabling the reduction to length of stay, ensuring people are on the correct pathway and receive the right care, in the place at the right time.
