This is a great opportunity for a Band 7 Clinical Complex Care Lead for Children & young people with Mental Health & Learning Disabilities, to bring about service transformation through improvement in the quality, design and delivery of commissioned services that includes Childrens Continuing Care, Transforming Care, joint packages and Personal Health Budgets across Coventry and Warwickshire?
The post holder will play a key role in delivery of person centred assessments in order to identify any unmet care need which may result in eligibility for continuing care, ensuring the National Framework for NHS Continuing care for Children and Young People is implemented and complies accordingly to the principles and processes with onward referral to other agencies where appropriate.
To monitor out of area placements and carry out quality oversight visits in line with the national guidance for people with a learning disability and/or are autistic. To ensure that 3 / 6 monthly / annual reviews are undertaken in an appropriate and timely manner and re-evaluate rationale for placement, appropriateness and funding
The role will ensure that the provision of care achieves individual outcomes and ensures the delivery of efficient quality packages of care that is aligned to the Personalisation and SEND agenda and supports the transition of young people into the appropriate adult services
To support the assessment & collate information from screening tools for children & young people with mental health & learning disabilities and to lead MDTs and reviews.
To advise the Personalised Care Lead on the outcome of assessment and to make recommendations i.e., decision/outcome of eligibility for continuing care.
To work in collaboration with Children Services to plan for well supported transitions into adult services, for young people with LD / ASD and complex needs.
The post holder will work in partnership with stakeholders and other professionals across both the public and private sector, education, health and social care partners to ensure a team around the child ethos for Continuing care is met. This would require liaison with the relevant health professionals, education, children with disability social work teams and other wider members of the team around the family
**Assessment and Clinical Component**:
Care Framework to individual’s healthcare needs to reach a decision with regards to
eligibility.
2. To act as a resource for all members of the multi-disciplinary team, providing expert
nursing advice concerning aspects of children & young people management.
3. The role will ensure that the provision of care achieves individual outcomes and
ensures the delivery of efficient quality packages of care that is aligned to the
Personalisation and SEND agenda and supports the transition of young people into the
appropriate adult services
4. To support the assessment and collate information from screening tools for children &
young people with mental health & learning disabilities and to lead MDT’s and reviews.
5. To undertake case management duties to meet the assessed needs of children &
young people who receive Childrens Continuing Care funding.
6. To co-ordinate the information and assessments required to enable the MDT to proceed
and to offer guardians / carers the opportunity to contribute information prior to the
MDT.
7. To promote joint working between agencies by recognising and acknowledging the
abilities and constraints of Health and Social Services in identifying and responding to
Children’s Continuing Care.
8. To advise the Personalised Care Lead on the outcome of assessment and to make
recommendations i.e., decision/outcome of eligibility for continuing care.
9. To lead and Chair MDT’s and to report the outcome as appropriate.
10. To ensure that 3/6/12 monthly reviews are undertaken in an appropriate and timely
manner and re-evaluate rationale for placement, appropriateness and funding.
11. To work in collaboration with Children Services to plan for well supported transitions
into adult services, for young people with LD/ASD and complex needs.
12. To monitor out of area placements and carry out quality oversight visits in line with the
national guidance for people with a learning disability and/or autism.
13. To alert safeguarding issues as appropriate and to follow up as required.
14. To alert the Personalised Care Lead with issues which are contentious or complex.
15. To escalate issues of concern to the Strategic Lead for Transforming Care in a timely
and succinct manner.
16. To support nursing audits, practice developments and staff development in Childrens
Continuing Care
17. To provide clinical expertise and advice to colleagues and be a role model.
