**Community Connector**

The Local Coordination Service operates in three geographical regions: Goole and Haltemprice, Bridlington and The Wolds, Beverley and Holderness. Each area is serviced by a Local Coordinator and a
**Community Connector**. You'll collaborate with the Local Coordinator, managing a caseload of individuals with unmet social needs. Using strength-based interventions, you'll help service users interact with community services and activities to enhance well-being.

The service operates in three distinct geographical areas:
Haltemprice and Goole x 1 vacancy

Bridlington and the Wolds x 1 vacancy

Beverley and Holderness x 1 vacancy

Our team supports people who are on the edge of care, or whose needs are escalating, to maintain their independence and improve their well-being. We respond to the needs of people who may be vulnerable, in crisis or struggling to navigate services, maintaining a network of external relationships across health, social care, emergency services and the voluntary community sector.

This role would suit someone who has excellent social and communication skills and has knowledge of how Health and Social Care services work. Are you ready to join our team and help spread the power of kindness?

**What does a day in the life of a Community Connector look like? You'll be**
- Liaising with partnership agencies to advocate for people affected by a range of unmet needs, social isolation and loneliness, to access services in order to reconnect with their communities, maintain or regain independence and improve well-being.
- Engaging in face-to-face with service users to identify specific needs and appropriate interventions.
- Ensuring that the service is efficient, reliable and delivered consistently with national and local guidelines, using the Quality Standards Framework as a basis for assessment.

**To be a successful Community Connector you'll**:

- Work as part of a team and on own initiative
- Have knowledge of the local area served by the scheme
- Plan and manage your own workload
- Have experience of lone working in the community
- Have experience of health and social care casework
- Know how to improve service quality for the benefit of users
- Have a willingness to work flexible hours
