Short Breaks Worker

The Stephen Wiltshire Centre is run by Hammersmith and Fulham council. It provides short breaks to children with disabilities and complex needs in Fulham including after school sessions, weekend short breaks and holiday provision. We also provide outreach in the Community for some children, as part of their short breaks packages.

We are currently recruiting to a number of posts:
Short Breaks Delivery Worker (2 posts - 18hrs) Come and support children to engage in play and development their independence. No two days are the same, from dinosaur golf to music festival parties, sensory play, arts and crafts and cooking. You will be skilled in supporting children with mobility and other physical needs and challenging behaviours. This role allows for development of skills and progress up the grade scale.

**Role Responsibilities**:
To provide safe and stimulating direct short breaks support to children and young people with disabilities and complex health needs, in a range of settings including SWC, in the community, in universal or mainstream clubs and in the child’s home.

You will:

- Deliver of a broad programme of quality, stimulating play opportunities including sports, games, arts, crafts, activities that are fun, creative and offer the children developmental experiences, this will include creative session planning and monitoring.
- Work on site at SWC, in the family home and in the community as part of a rota, this may include early morning, day, evening, and weekends as needed
- Shift lead sessions as part of the rota.
- Provide personal care, as per the child’s individual care plan and any other individual plans for example: manual handling, risk assessments, feeding, medicine administration and management, suction or behaviour plans
- Be a key worker to allocated named children as requested, being the family contact in the service and responsible for maintaining accurate up to date records for key children
