Healthcare Support Worker

**Job summary**:
Are you looking for an exciting role in a supportive team in an expanding service?

Due to the exciting expansion of the In-Reach into Care Homes service we are currently seeking a skilled, motivated, health care support worker with a desire to provide high quality and timely care under the supervision of a qualified practitioner.

Working with older people who live in a care home and have mental health needs, you will be passionate about working with older people and committed to improving their quality of life.

You will need excellent communication skills preferably with experience of working with people with dementia. You will need to work independently but under the supervision of a senior member of staff supporting the care who reside in care homes and have dementia or other mental health difficulties and may be presenting with distress.

You MUST be able to drive and have access to a car during working hours.

**Main duties, tasks & skills required**:
1.1.To provide physical and psychological care to service users in accordance with the specified plan of clinical care.

1.2.To carry out duties as designated by the person in charge

1.3.To provide support to the nurse in charge and/or the person responsible for the clinical care of the service user

1.4.To accurately record patient/client information

1.5.To provide support to relatives/carers

1.6.To carry out triage calls as directed by senior colleagues

**Essential**:

- Literacy and numeracy skills
- Completion of NVQ Level 3 in Health and Social Care
- Previous experience especially of working with people with Dementia.
- Computer skills
- Clean driving license and access to own vehicle during working hours.
