Pennine Care NHS Foundation Trust are delighted to be developing a Community Rehabilitation Team across the trust footprint.

An exciting opportunity has arisen to be a part of the Multidisciplinary Team as Occupational Therapist. Working in a flexible /hybrid approach the post holder will provide evidenced based care and play an an integral role in delivering a recovery focused service as part of a whole system approach.

The multidisciplinary team will provide specialist clinical care and work within an overall framework for the delivery of community mental health services to support people with complex psychosis who are living in funded placements, including care homes and supported accommodation. Incorporating the NICE guidelines for complex psychosis and supported by the Getting It Right First Time (GIRFT) national program

Care coordination - To manage a defined clinical caseload, using evidence based and client centered ,
principles to assess, plan, implement and evaluate interventions in inpatient and
community settings and on home visits.
To maintain clinical records that are fit for purpose
