Health and Wellbeing Coach

We are looking for a an experienced and passionate Health and Wellbeing Coach to join our Prisons & Criminal Justice team in Durham & Cleveland.
**About the Role**
You will work as part of a multi-agency team, delivering high quality person centred support focussing on personal wellbeing issues for those leaving the prison settings in Durham and/or Cleveland area, in line with the new Dynamic Probation Framework model. I will provide person centred support to a caseload of service users, who will either be serving community sentences or being released from prison, in group and 1:1 formats, in a responsive, flexible and adaptable way, working with other professionals to achieve the best possible outcomes.
**Key Responsibilities**
Provide person-centred support to individuals transitioning from prison settings or serving community sentences.
Facilitate group sessions and 1:1 support in a responsive, flexible, and adaptable manner.
Address personal wellbeing issues and guide service users toward positive changes in their lives.
Work collaboratively with professionals from various teams to deliver seamless support and ensure optimal outcomes.
Build and maintain strong working relationships with service users and stakeholders, providing guidance and encouragement throughout their journey.

- **Appointments will be at the minimum of the band.**
- **London Allowance is paid for roles that are based within the Greater London area only.**
