To be a core member of cancer site-specific multi-disciplinary team/s, providing full administrative and information support in all aspects and business activities of the multi-disciplinary team/s. The co-ordinator will be required to collate, record and validate data for submission to national (UK wide) and/or Wales wide based audits, including Cancer Peer Review and Cancer Standards and any local audits identified by the multi-disciplinary team/s. Ensuring submission documentation is complete within the specified deadlines.
To actively ensure that all patients are treated within National Targets set for cancer patients. Ensuring all MDT members are aware of patient’s wait status and target dates during MDT discussions, proactively taking steps to identify delays and escalate to the clinical team and relevant operational management teams to ensure that patient’s treatment schedules are brought back within target.
Manage the co-ordination of Cancer Site Specific MDT Meetings. The co-ordinator will collect all essential information as determined by the relevant data set. This requires the co-ordinator to work closely with the clinical team.
To record the outcome of each and every patient listed for discussion at the MDM (multi-disciplinary meeting) on the relevant database.
Be responsible for and ensure that any appropriate follow-up actions from MDT meetings are carried out as identified during the discussion, e.g. Patient requires a CT scan, check request have been made and re-book for MDT when appropriate
In order to facilitate the efficiency of the MDT meeting, it is the co-ordinators role to organise the key elements required, e.g. radiology and pathology reports are ready in order to ensure that the information is available at each meeting.
To challenge the progress of patients’ journey with all members of the cancer MDT
Take responsibility for running regular data quality and validation reports from the relevant database.
An extensive knowledge of all the cancer sites and other team dynamics is required in order to cross cover for other MDT co-ordinators. To keep an adequate preparation guide to be used in the post holder’s absence.

**KEY Responsibilities**:
1. Manage the co-ordination of Cancer Site Specific MDT Meetings which may include a pre MDM triage meeting. As a core member of the MDT the co-ordinator will collect all essential information as determined by the relevant data set. This requires the co-ordinator to work closely with the clinical team, highlighting information deficits to ensure completeness of data.
2. The co-ordinator will use the Canisc MDM Module to record the outcome of each and every patient listed for discussion at the MDM (multi-disciplinary meeting). Following the MDM, the co-ordinator, as means of communication with the GP/referring clinician and onwards referral will, in a timely fashion, produce a letter from Canisc, which will include the outcome and recommendation of the discussion.
3. Be responsible for and ensure that any appropriate follow-up actions from MDT meetings are carried out as identified during the discussion, e.g. Patient requires a CT scan, the co-ordinator will check that the request has been made and being progressed in a timely manner and a follow-up discussion arranged if required.
4. In order to facilitate the efficiency of the MDT meeting, it is the co-ordinators role to organise the key elements required, e.g. the requesting and collection of case notes, radiology and pathology reports in order to ensure that the information necessary for effective team functioning is available at each meeting. In addition, the role also requires the post holder to arrange the availability of venues and catering requirements as well as formally setting up the equipment and seating arrangements at the venue.
5. At a frequency determined by the Health Board Cancer MDT Operational Policy, support the MDT lead to organise and facilitate all business meetings by arranging venues, sending invitations, circulating agendas and relating documents. T
