**Job Title: Care Coordinator**

**Location: Poole/Bournemouth/Ferndown**

**Salary: up to 27K Per Annum**

**Hours: Variable. Weekends..Every Other**

Bespoke care is looking for an experienced driven, dynamic **Care Coordinator**who has worked within the domiciliary care sector and has experience of coordinating within domicillary care. We are a small but ever growing care buisness who welcomes an iundividual who has a passion for delivering outstanding care but also a desire to grow our buisnees.

Your working week would be Monday - Friday office opening times plus out of hrs support, you would also be available to work alternate weekends to support with out of hours on call.

**Brief Description**:

- To plan and schedule clients care calls to meet their individual needs and requirements and as specified by their individual care and support plan. To structure individual staff availability into the client call times so that continuity is maintained to support client’s needs.
- To ensure quality care is maintained and monitored throughout the service delivery through effective rostering and develop professional working relationships with professional bodies and partners.

**Duties and Responsibilities**
- To plan daily rotas to meet clients’ needs and requirements
- To structure and plan runs to support staff availability
- To liaise with local authority brokerage teams to build and develop new rounds for growth and staff members’ availability as well as maintain existing rounds.
- To ensure all care calls are allocated to a member of staff and put onto a master weekly working plan.
- To ensure staff receive their working rota’s in a timely manner so to prevent miss communication and missed calls.
- Likewise, to ensure all clients receive a weekly schedule so to reduce anxiety and provide comfort and reassurance as to who is coming and when.
- To Liaise with Bespoke Cares management team and careres in planning group staff meetings.
- To conduct staff supervisions when due and back to work interviews in line with company policy and procedure.
- To support other locality coverage should a member of the care coordinating team become absent.
- To support other departments in busy periods - work as 1 team.
- To carry out telephone calls or visits to clients to offer support and reassurance should they have any concerns relating to their schedules.
- To deal with complaints relating to schedules.
- To ensure compliance within the recruitment process and support the Registered Manager within the induction process for new starters.
- Coordinate training sessions, ensuring all logístical aspects are managed effectively.
- Maintain accurate records of training activities and employee progress in compliance with company policies.
- To support and hold our on call phone on a rota basis within the management team.
- To be flexible to adapt to any changes that may occur within the company for the good and benefit of the company.

**About you**:

- Experience of working in Care Coordination environment
- You will be able to work alternate weekends
- You will be computer literate and familiar with all word and excel programmes as well as outlook. Full training on the use of our computer rostering system will be provided.
- A good command of English language both written and spoken and the ability to converse with both care workers, professionals as well as clients and their families is essential.

Pay: £24,778.00-£26,161.00 per year
