**key Responsibilities**: 1. Clinical Leadership: Lead the clinical care provided by the Enhanced Care Team, ensuring high standards of patient care and safety. Utilise advanced clinical skills and expertise to manage complex patient cases and provide guidance to team members. Collaborate with the multidisciplinary team to develop and implement comprehensive care plans tailored to meet the individual needs of patients.
**2. Managerial Leadership**: Provide managerial oversight and direction to the Enhanced Care Team and the Integrated Neighbourhood Team. Foster a collaborative and supportive team environment conducive to professional growth and development. Oversee the allocation of resources and workload management to ensure efficient and effective service delivery.
Engage with and use Public Health Management Data to identify patients in the greatest need and target resources accordingly. 3. Relationship Building: Cultivate strong working relationships within the PCN and PCN constituent practice, social and healthcare providers, including local authorities, voluntary organisations, and healthcare facilities. Collaborate with the PCN Management team and external stakeholders to coordinate care pathways and facilitate seamless transitions of care for patients.
Act as a liaison between the PCN and external partners to advocate for the needs of patients and optimise service delivery. Advocate for the needs of Brunel PCN patients, ensuring equitable access to healthcare services and resources. 4. Patient-Centred Care: Work collaboratively with patients and their families to develop personalised care plans that promote holistic well-being and self-management.
Advocate for equitable access to healthcare services and support initiatives aimed at reducing health inequalities within the community. Engage with and use Public Health Management Data to identify patients that are in the greatest need. 5. Quality Improvement: Lead quality improvement initiatives within the Enhanced Care Team and the Integrated Neighbourhood Team to enhance the delivery of care and patient outcomes.
Monitor performance metrics and outcomes data to identify areas for improvement and implement evidence-based practices. Participate in clinical audits, peer review processes, and service evaluations to ensure compliance with standards and guidelines. 6. Professional Development: Stay abreast of developments in advanced clinical practice, healthcare policy, and best practices relevant to the role.
Engage in continuous professional development activities to enhance clinical knowledge, leadership skills, and managerial competencies. Serve as a role model for professional behaviour and uphold the highest standards of ethical conduct and accountability. Overall, the Advanced Clinical Practitioner Enhanced Care Team Lead will play a pivotal role in driving excellence in patient care, promoting collaboration across healthcare sectors, and advancing the strategic objectives of the Brunel PCN in delivering integrated, high-quality services to the community.
