The Pembrokeshire Community Falls Team has a vacancy for a Falls Assistant Practitioner to support the delivery of a Falls Prevention and Management Service to our Pembrokeshire population.

The post holder must be computer literate, have good organisational skills and be committed to consistently delivering a quality service.

This role will require a flexible approach to work, with an ability to prioritise and manage workloads. Individuals will be able to work as part of a team and also under their own initiative.

The post holder will be working within the Falls Team, which consists of two Clinical Falls Practitioners. The role will support the delivery of effective, co-ordinated care for vulnerable and frail adults at risk or who have a history of falls.

You will be responsible for delivery of falls prevention and management sessions to public groups, staff, volunteers in the 3rd sector and community organisation groups.

You will carry out multi-factorial home risk assessments and falls prevention interventions for people who have fallen or at risk of falls. This will be either by yourself or with the Clinical Falls Practitioners.

A flexible and agile approach to work is necessary. The post will be based in South Pembrokeshire Hospital in Pembroke Dock and will include a balance of office and home working. You will need to be able travel around Pembrokeshire as part of your role.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
